Temple Border (Aquarfall Marsh) — choose [Combat] to help the shadowy figure
Choose The Void at the end of Act III, go to the Plaza and pick the [Enter] option to use the Orb — this opens the Withered Vale. Fight through to the Withered Soul and defeat the boss; Althea unlocks in the aftermath.
